File a Complaint

If you wish to file a complaint against a psychiatrist who is a member of the Panel of Psychiatrists, please submit your complaint via this online form: Complaint Form | FormSG.
Before you submit your complaint, please note that:
The Panel was established to safeguard the objectivity and quality of psychiatric expert evidence. Only psychiatrists who are admitted to the Panel may give evidence on a matter concerning psychiatry in criminal proceedings in Singapore.
Your complaint should be limited to complaints about a psychiatrist’s eligibility to remain on the Panel and to give expert evidence in criminal proceedings in Singapore. The eligibility requirements are set out at Apply for Appointment | Panel of Psychiatrists
You will be required to upload a statutory declaration that is made in accordance with the First Schedule of the Oaths and Declarations Act 2000. If you do not have the necessary statutory declaration, please obtain one before submitting your complaint.
After the Selection Committee considers your complaint, the Selection Committee may (a) start revocation proceedings against the psychiatrist; or (b) dismiss the complaint. You will be notified of the Selection Committee’s decision.
